Membership of the club is confined to Amateurs, defined by the United Kingdom Athletics (UKA) rules of competition. Fleet & Crookham Athletic Club (F&CAC) is a Charitable Incorporated Organisation, registration number 1156074. Membership details are stored electronically but not distributed beyond the committee, coaching team and organisers of F&CAC events and training sessions:
o I have read, understood and accept the club’s Privacy Notice
o I agree to my e-mail address being used for club communications (This must be the parent’s or legal guardian’s email address).
o I consent to my child’s name, age category and gender being displayed publicly on the results page of F&CAC website.
o I give permission for photographs of my child undertaking athletic training and competitions to appear on F&CAC’s website and Facebook page.
o I have noted all medical conditions on this form including any disability that could affect participation of your child in training sessions and will notify the relevant coach before each coaching session to allow safe running of training sessions (the club recommends all members consult their Doctor before participating in club training sessions).
o I acknowledge that my child participates in all events at their own risk.
o I agree that F&CAC and its Trustees shall not be liable for any accidents, injury, loss, damage or claim or expense that may arise in consequence of my child’s participation in club activities/events.
o I agree I have parental responsibility and therefore sign on their behalf.
When your child becomes a member of or renews their membership with Fleet and Crookham AC, they will automatically be registered as a member of England Athletics and be provided with a Unique Reference Number (URN). We will collect certain information about your child which will include their name, date of birth, gender, URN, parent’s or legal guardian’s email address, home address, parent’s or legal guardian’s telephone number, names of any other clubs that your child is a member of and details of any coaching or officiating licenses they hold. We will provide England Athletics with their personal data, which they will use to enable you to access an online portal (called myAthletics). England Athletics will contact you to invite you to sign into and update your child’s myAthletics portal data (which, amongst other things, allows you to set and amend your privacy settings). If you have any questions about the continuing privacy of your child’s personal data when it is shared with England Athletics, please contact dataprotection@englandathletics.org.
I acknowledge that my liability to the club on behalf of my child continues until a written resignation is received and acknowledged by the club secretary.
